An internal bra lift is a type of breast lift that includes the added reinforcement of a mesh material, adding internal support and more elevation at the lower pole of the breasts. Do I Need a Breast Lift?

A breast lift is suitable for just about any woman who has found that her breasts sag lower than they used to. There are a variety of reasons this occurs; weight fluctuations, pregnancy and breastfeeding, other hormonal changes, and the natural aging process can all take a toll on the breasts. This can be attributed to the weakening and degradation of collagen fibers within the skin, causing more downward migration and stretch marks. The Cooper’s ligaments- the connective tissues that suspend the breasts at a higher positioning- can also lose their recoil, failing to suspend the breasts at a higher positioning. (1)(2)

Sometimes, women may undergo a breast augmentation with implants, but find that their tissues are not strong enough to support breast weight over the years. Sometimes, irregularities in how breasts heal or a surgical mistake with the dissection method allows the implant to “bottom out” or result in a phenomenon called “double bubble”. While “bottoming out” refers to breasts that have an abnormal amount of volume in the lower pole of the breast mound, “double bubble” refers to a problem with the surgical placement of the inframammary fold, making both the implant and natural breast tissue migrate downward at different locations like two “bubbles”.

With a standard breast lift and/or breast implant revision with lift, Dr. Bonnor can fix a wide array of issues by repositioning the breast mound, forming a natural-looking inframammary fold, and removing excess skin. In certain cases, however, he may insert the mesh using the same incisions to prevent sagging and other aesthetic issues in the future. The insertion of the mesh only adds about half an hour of procedure time to the lift/revision surgery.

Implant Removal or Exchange

If you enjoy the look of your implants, Dr. Bonnor can perform a variation of the procedure that includes implant removal and the application of mesh. To begin, Dr. Bonnor will first administer the appropriate anesthesia and make an inframammary incision that follows along the bottom fold of the breast. If you have saline implants, he will utilize a surgical syringe to drain the contents of each implant before removing the silicone shells. For smaller implants, he may be able to remove them using periareolar incisions (surrounding the areola) or a keyhole/lollipop incision that includes a periareolar shape alongside a vertical incision from the areola to the lower fold of the breast.

Breast Capsulorrhaphy and Total Capsulectomy

Dr. Bonnor may need to revise the breast pocket and/or capsule where the implant lies. This may include a breast capsulorrhaphy, a procedural technique that includes reshaping the capsule with sutures after placing a replacement implant. A total capsulectomy is required for those with capsular contracture, a condition in which too much tight scar tissue forms around the implant. With a capsulectomy, Dr. Bonnor will remove the fibrous capsule and the implant within as one unit. He can then insert new implants immediately afterward if you desire.

Internal Bra Lift without Implants

If you do not have implants and do not desire them, Dr. Bonnor can restore an all-natural look and feel with a breast lift and mesh if he feels that you would benefit from it. Without the extra steps of implant removal, Dr. Bonnor can begin by making periareolar incisions (for mild ptosis), keyhole/lollipop incisions (for moderate to severe ptosis), or anchor/wise-pattern incisions (for severe ptosis). He can then adjust and insert the mesh so that it becomes the perfect internal scaffold, no matter the lift technique. After excising and tightening the skin and repositioning the breast mounds, he will securely suture the incisions and apply a surgical dressing.

The Regnault Classification System

To get an idea of what kind of technique Dr. Bonnor may prescribe, you need to understand your level of breast sagging/ptosis with the Regnault Classification System:

  • Mild ptosis is characterized by the NAC being at the same level as the inframammary fold.
  • Moderate ptosis means that the NAC has fallen below the inframammary fold, but it is still level with the lower breast contour
  • Severe ptosis is when the NAC lies below the inframammary fold and the lowest breast contour, and it points downward.

Commonly Asked Questions (FAQ)

Will I be able to feel the mesh from the outside?

Until the internal bra fully dissolves after 12 to 24 months, you may be able to feel it with your fingers from the outside, but you will not be able to see it. Usually, women with less tissue in their breasts will be able to feel the scaffold. It is not a problem and is a normal part of the healing process after a breast lift.

Can the mesh fit breasts of any size?

Yes, mesh comes in various sizes and rectangular, oval, and circular shapes. It is also available as a curved “3D” shape that serves to surround the lower pole of the breast in a more rounded fashion.

Can the mesh move around internally?

Mesh is sutured to the lower portion of internal breast tissue, so the material will not move around or displace itself.
